One of the burnout games, all F1 cars, the really low ones with fins, was in first place with everyone right on me going into a turn near a tunnel, screen flashes, all i see are tires rolling through the tunnel, everyone wrecked hard enough to cause an avalanche of tires to flood into the tunnel, totally my bad...

While playing some Diablo 3 on the computer, and running some rifts, I was using a build that utilizes a teleport function and in Diablo you can teleport through walls to like the next room. I teleported in a group of elites with the worst buffs(arcane: spawns a death orb that murders you, molten: leaves fire wherever it walks and when it dies it explodes, and plagued: spawns pools of poison to do damage over time) and I couldn't escape because the teleport needed to cool down so I got destroyed in like 0.5 seconds... I stood no chance >.<

I think for me it'd be that time when me and my brother would play Super Smash Brothers Melee. I was really close to finally beating him, when I killed myself by using a downwards smash attack.

first one that comes to mind was fairly recent, criminal mastermind challenge in gta 5(4th heist) i was arguing with my friend who kicked me out of his car in the middle of the highway and i was like "whatever i'll just jack a ride" i then proceeded to get run over about 6 times before a rig hit me for the killing blow costing us the challenge. And yes i was trying to avoid the cars when i saw my health get low but the gta gods had other plans

Final Fantasy 13.  I ran around for 2 hours trying to find the way to go when it was literally right in front of me, and I had passed it like 20 times.  It was a sad day for me then.

Forgot there was an ignition bomb in my vehicle on GTA 5. And I poisoned myself on Oblivion by accidentaly eating a poison apple. I also set my controler down on Dark Souls and hit the firekeeper. She never forgave me. In fact, she hated me so much, that she fell to her death trying to kill me.
